Duro-Last PVC Protective Walkway Pad Issues

Has anyone by chance ever experienced any problems/issues with welding/securing Duro-Last PVC protective walkway pads only to have them eventually break loose from the Duro-Last PVC roof membrane surface beneath? While the Duro-Last PVC side lap seams and roof membrane flashing appear to be in overall good repair and weather tight for a PVC single-ply roof installed back in 2016, the PVC walkway pads are not. Any comments/suggestions regarding this subject matter are most appreciated. While I do have photos to show what's happening, the forum does not appear to provide a way to upload them. Thank you ahead of time.

Sounds as if the membrane wasn't cleaned proper during installation. If you could try again, photos would be nice.
